Creating a Chronic Disease Management Plan
Living with a chronic illness often means adjusting your lifestyle, medications, and ongoing care. At Trinity Healthcare Centre, your GP will work with you to develop a Chronic Disease Management Plan tailored to your individual needs. This plan will:
- Identify your current health concerns and long-term care goals
- Outline the services your GP will provide directly
- Include referrals to relevant allied health professionals and specialists
- Map out clear steps that you and your GP agree on to support better health outcomes
Once your plan is in place, your GP will schedule regular reviews to monitor progress and adjust your care as needed. These follow-ups are key to catching any changes early and ensuring that your treatment remains appropriate over time.
We also work closely with trusted allied health providers—including physiotherapists, podiatrists, dietitians, occupational therapists, and mental health professionals—to support every aspect of your condition, all under a coordinated care model.
