Chronic Disease Care Plans
If you’re living with a chronic condition, you may be eligible for a GP Management Plan through Medicare. This plan helps you set clear health goals with your doctor and access up to five Medicare-covered visits per year to allied health professionals like physiotherapists, podiatrists, psychologists, dietitians, and more.
Your care plan is reviewed every 6 months and updated every 2 years. There’s no cost to you, though some providers may charge a small gap fee.

Health Assessments
We offer government-funded health checks to support your long-term wellbeing.
Over 75s Health Assessment
Available yearly for those aged 75 and over. This comprehensive check reviews your overall health, medications, lifestyle, and any support needs at home. Covered by Medicare and takes around 1 hour.
45–49 Year Old Health Check
A once-off assessment to identify risks and help prevent chronic disease through early lifestyle changes. Recommended for those at risk due to family history or lifestyle factors.
All assessments are bulk billed and include a nurse-led consultation followed by a GP review.
